Our office is proud to offer you the most up-to-date treatments in allergy, natural hormone therapy, nutritional support, and natural health support, including supplements and medical foods.

Our facility is located in Bloomingdale, near Stratford Mall on Gary Avenue, one block north of Army Trail Road. Take Gary Avenue to Scott Drive, turn east and our parking lot is the first driveway on the right.

We allow up to 60 minutes for an initial consultation (including chart/records review). In order for you to get the maximum benefit from your appointment, we ask that you complete all necessary paperwork ahead of time and still come 10 minutes early to your appointment to allow the clinical staff the time they need. If you arrive late to your new appointment, we will do what we can with your remaining time so that we do not inconvenience later appointments.

Now that you’ve taken the first step on your road to vibrant health by making an appointment, sign in to the Patient Portal. We’ll be happy to answer any questions you may have before your visit.

It is helpful if you bring any recent labwork (within a year) and test results that may be significant.

We also ask that you do not wear any scented products to our office as it interferes with allergy testing and is bothersome to our chemically sensitive patients.

Starting in January 2023, services will need to be paid at the time of service which include copay, coinsurance and/or allowable fees.

We highly recommend you contact your insurance company or plan to confirm in-network status contracting with Dr. William Epperly – NPI 1477641645.

 

We do NOT bill to any HMO insurance plans, Medicare, Medicaid, secondary insurances or third party payors, including workman’s compensation.

 

Unless we are contracted with your insurance, payment in full is due at the time of service.

 

Copays are due at the time of visit. Patients will receive an explanation of benefits from their insurance company outlining payments, discounts, and any patient responsibility for services rendered in our office. The patient portion due to our office is due immediately and is not contingent on receiving a bill from our office.

 

Note: when you leave from your appointment you will be given a walk-out statement that has sufficient information for you to bill your own insurance (if we are not contracted).

Dr. Epperly was originally trained as a Family Practitioner over 35 years ago. He has evolved into an integrative and functional medicine doctor using a variety of methods to prevent and cure disease including functional, lifestyle, and nutritional medicine principles. He also has special expertise and interest in male and female hormone replacement, environmental allergy, and hypothyroidism, although he is not board-certified in these areas.​  Since we are no longer your routine Primary Care practice, we encourage patients to have a regular primary care doctor to handle routine acute care issues and school and sports physicals and the like.  But our practice still includes routine woman care provided by Anjitha and Roanne.   We no longer see children for routine child care, but we do see them for chronic health issues or allergies.  We recommend that any children we do see for chronic health issues also have a pediatrician or family doctor for routine care.  We see younger children for chronic health problems on a case-by-case basis.

PA’s are licensed to do almost everything the doctor can do. Our PA’s share Dr. Epperly’s philosophy on integrative healthcare, hormones, and functional medicine and have been trained by him to ensure continuity in care.

Yes, IV therapy of several types is available.

No, by contracting with us directly federal law prohibits you from billing Medicare and therefore secondary insurances generally do not apply.

Yes, Northwestern Medicine Central Dupage Hospital in Winfield. But he does not practice any hospital medicine. If you do happen to need hospitalization, your in hospital care at Central Dupage will be managed by one of their highly qualified “hospitalists” who are physicians who specialize in inpatient care or else by specialists.

Contact your old physician’s office and find out their policy on releasing records. You will likely need to fill out a records release form that they can provide.

Please call the office and speak with an office member who will try to help you. If necessary, your message will be routed to Dr. Epperly for follow up. If your problem or question involves medical management, Dr. Epperly does provide phone consults at a cost of $60.00 / 15 minutes. Time includes chart review, writing prescriptions, mailing/faxing, etc.  You can also message our office through our patient portal.

You will be given a free copy of your office notes and labs when you come in to see the doctor. There is a fee for additional copies. You must fill out a release of records request form provided from our office.

Dr. Epperly’s practice provides services in a consulting role. This means that we are only available for scheduled appointments during normal office hours. We request that you have a primary health care provider for urgent care and after hour needs. Our office does not offer “on call” services

Dr. Epperly nor his PA’s require a referral to see you as a patient. All you need to do is contact our office to arrange a new patient appointment at 630-893-9661. We will put all your information into our EMR system, send you the new patient forms and put you on the “pre-scheduling” column. Once all the new patient forms are filled out and your information is in our system, we will move your “pre-scheduled appointment” to an actual appointment with one of the providers. Note: you will need to provide your insurance card and credit card information to our new patient coordinator when making a new patient appointment. 

In order to best service our patients, it is our policy to require two (2) business days for appointment rescheduling and cancellation. For example, an appointment on Wednesday, should reschedule/cancel no later than the previous Monday to avoid a late cancellation fee of $75.00. Cancellation fees are NOT covered by insurance. There is a “no-show” fee of $75.00. Late cancellation/no-show fees must be paid prior to receiving future services. This will allow us to best service all patients in all capacities.
